Overview

Responsible for coordinating the SUPPORT Program Grant, which is focused on substance use disorder services for individuals or groups. This is a grant-funded position that reports to the System Director of Community Health.  Grant is expected to last approximately 18 months.

Responsibilities

      • Oversees State of Illinois Healthcare & Family Services SUPPORT Program grant.
      • Participates and coordinates the budgeting process for the SUPPORT grant.
      • Maintain regular communication with the grant funder and serve as their primary point of contact.
      • Evaluates the process, outcomes, and ROI of MAT (Medication Assisted Treatment) and substance misuse prevention and counseling initiatives by utilizing a variety of measurement tools and EHR data.
      • Oversees the development, planning, management, and reporting process for SUPPORT grant funding.
      • Responsible to ensure the SUPPORT Project is being implemented in compliance with grant requirements, as well as established SIH policies and procedures.  
      • Responsible for reporting on project activities and for reporting all other operational and administrative issues.
      • Conducts surveys of physician needs related to Medication Assisted Treatment (MAT).
      • Collaborates with physician and mid-levels to offer MAT training and to provide technical assistance and coordination opportunities.
      • Attends SIH and Community Coalition Pain Management Meetings related to opioid use.
      • Maintains all required records, reports, statistics, logs, files and other documents as required.Manages processes effectively in regard to employee/patient safety
      • Promotes a culture of process improvement by participating in unit/department based programs that supports the system’s process improvement goals.
      • Actively participates on system-wide or hospital-based teams as needed.
      • Manages processes effectively in regard to employee/patient safety. 

     

    Qualifications

    Education

    • Bachelor’s degree in human services field such as Public Health, Mental Health/substance use disorders, Social Work, Education or related field

     

    Experience and Skills

    • Technical Experience: 3 years in program development and management in delivering human services, with a focus on substance use disorders and/or experience in administering programs and policies related to behavioral health services.
    • Administrative skills in data and survey collection
    • Performance management and grant implementation knowledge and experience required.
