  • Overview

    Responsible for assisting hospital Case Managers with clerical work related to referrals to nursing homes, home health agencies, durable medical equipment companies, and other community resources. Additional duties related to providing patients with the important message from Medicare and information packets regarding POA may be required.


    Roles and Responsibilities
    o Receives and facilitates referrals for routine home health needs, durable medical equipment and supplies from hospital staff or Case Manager
    o Assist Case Managers with arrangements for NHP or return to NH.
    o Refers appropriate patients to case management or health management Coordinator if discharge needs are complex.
    o Provides assistance to the Case Manager by communicating via telephone or facsimile to agencies, compiling reports, data entry, and other functions as needed.
    o Communicates with third party payers regarding pre-approval requirements and preferred provider identification for discharge planning needs and for guidance in referral to appropriate agency and communicates this information to the appropriate agency.
    o Gathers information from laboratory, radiology, or other areas of the hospital to facilitate movement of patients to levels of care or discharge.
    o Facilitates discharge planning for patients in the acute care setting which includes completing arrangements for transportation.
    o Performs clerical functions related to data collection for utilization review
    o Keeps Case Manager and team involved as to the status of arrangements and any barriers or obstacles encountered.


    High School diploma or equivalent required

    Associate's degree preferred


    Experience and Skills
    Technical Experience: 1 year specialized training
