  • Overview

    Responsible for developing a health coaching relationship with employees and assisting employees through the process of actively working towards better health.

     

     

    Responsibilities

    • Contacts participants that are eligible for the program for enrollment and provides necessary health coaching to reduce or eliminate high-risk behaviors.
    • Encourages adoption of habits that are conducive to a higher quality of life.
    • Supports operational aspects of the division to meet the organization's customer requirements and satisfaction.
    • Develops and conducts onsite health-related wellness seminars and training courses for interested employees and family members.
    • Develops and distributes health education materials to identified employees and family members.
    • Develops customized care plan for program participants including goals and plan of action.
    • Coordinates care with other health-related vendors for optimal patient care.
    • Designs and implements health education programs designed to increase awareness and knowledge for the improvement of health in our employees.
    • Keeps up-to-date on changes in health care technology to keep health education program material current.
    • Maintains security, confidentiality and integrity of database, including all participant data.
    • Demonstrates flexibility in meeting participant scheduling requirements.
    • Assists in evaluating the effectiveness of existing program.

    Qualifications

    Registered Dietician or Registered Nurse with a BSN required.
