The Population Health Disease Clinical Analyst (PHDCA) works to identify patients’ unmet needs, engage patient in their own care, gather summary information for treatment interventions and enhance ongoing communication between the patient and her/his care team. PHDCA gathers, analyzes, and disseminates data integral for the provision of preventative services and chronic disease management.

 

$63,980.00/yr minimum - $95,977.00/yr maximum (range increases based on education, experience and certifications)

Safety

  • Manages processes effectively in regard to employee/patient safety.

Record Keeping

  • Maintains all required records, reports, statistics, logs, files, and other documents as required.

Process Improvement

  • Promotes a culture of process improvement by establishing unit/department-based programs that support the system’s process improvement goals.
  • Collaborates with PH to establish culturally sensitive, population-based interventions and workflows.
  • Monitors and corrects population registries under the direction of the PH Medical Director and System Director.
  • Actively participates in system-wide or hospital-based teams.

Role Specific Responsibilities

  • Manage patient registries and provide members of Population Health (PH) and health care teams with the data required to meet the program's needs.
  • Provide data management, coordination and patient outreach as needed for specific target patient populations.
  • Assist in process mapping and development of workflows for PH Management.
  • Recognize and report data inconsistencies to appropriate personnel.
  • Uses nursing knowledge and expertise to drive care coordination efforts for patients with diabetes, and other chronic diseases/diagnoses related health management needs.
  • Help mentor new hires and assist in cross coverage as needed.
  • Maintains multiple caseloads/deadlines independently and promptly while responding to quality and other outcome-related measures.
  • Communicates with Care Team members to coordinate care.
  • Conducts concurrent medical record review using specific indicators and criteria selected for care gap reporting.
  • Actively collaborates with providers to close care gaps for value-based contracts.
  • Assists with orders for laboratory and diagnostic testing as appropriate per department clinical guidelines.
  • Participates in department huddles, meetings, and other meetings as appropriate.
  • Partners with Community Based programs to promote screening services.
  • Works under the supervision of the Supervisor and Director of Clinical Operations - Population Health.
  • Facilitates the SIH vision for population health at the system and local level to promote improved data acquisition and use in the continuous improvement of clinical and operational outcomes.
  • Serves as a resource at the system and ambulatory care levels, communicating population health care trends and issues at the local, regional, state, and/or national level.
  • Incorporates research and evidence-based knowledge into informatics practice.
  • Collaborates to identify educational opportunities.

Qualifications

 

Education

  • Associate degree, bachelor’s degree preferred in medical related field.

Licenses and Certification

  • Illinois Registered Nurse license

Experience and Skills

  • Technical: Minimum of three years’ experience in the use of Electronic Medical Record systems – EPIC preferred.
  • Proficient in data management and reporting.
  • Commitment to accuracy and integrity of all data, reports, and communication.
  • Proficiency in using web-based meetings and communications, Excel, Word, PowerPoint, and other software applications.
  • Ability to regularly commute to any SIH location within the Southern Illinois region for face-to-face interactions deemed critical to operations.
  • Technical Experience: minimum 6 years clinical experience in primary clinical discipline with primary duties working with clinical information systems within a healthcare setting required.
  • Must be able to type effectively and efficiently take accurate meeting notes.
  • Must have familiarity with Network, personal computer, communications, and database systems.
  • Must have familiarity with data privacy practices in healthcare.
  • Excellent relationship building and professional communication skills with internal and external customers.
  • Demonstrates ability to organize and coordinate functional teams and coordinates work to effectively accomplish project goals.
  • Liaises with customers, other ITS staff and vendors for design, planning, implementation, and problem resolution using effective communication, reporting and appropriate follow-up.
  • Sets and maintains goals, timelines, and commitments, demonstrating proven attention to detail and high level of organization and accuracy.
  • Provides innovative solutions to complex issues.
