$/hr minimum - $/hr maximum (range increases based on education, experience and certifications)

 

Responsible for coordination of care of CT Surgery patients, compliance with system protocols, facilitate CT surgery task force meetings and database entry.

 

Responsibilities

Role Specific Responsibilities

  • Coordination of Care
  • Transcatheter valve and Watchman coordination.
  • Provide care coordination during pre-op period and patient education
  • Liaison for patient and family during encounter
  • Navigation for evidence based practice during stay and post-op calls.
  • Facilitate Task Force Meetings:
  • Facilitate Structural Heart Task Force meetings utilizing evidence-based practice.
  • Collaborate with Structural Heart Team including CT Surgeions, cardiologist, Prairie Heart Structural Heart Coordinator, Cath Lab staff, nursing staff, case management, cardiac rehab, and physical therapy.
  • Address quality issues and lead process improvement.
  • Data Entry:
  • Enter data into LAAO and TVT Registries
  • Available to provide back-up data entry to other registries and care coordination as required.

Qualifications

Education

  • A.D.N or B.S.N. in nursing

Licenses and Certification

  • Current Illinois Registered Nurse

Experience and Skills

  • Technical Experience: 5 years RN experience in an acute setting
