$29.47/hr minimum - $44.21/hr maximum (range increases based on education, experience and certifications)

Position Summary
• Responsible for the day to day activities of case management. Performs
concurrent patient assessment related to appropriateness of the level of care,
diagnosis, procedures performed, discharge needs, and intervenes to expedite the
patient’s plan of care.

Education
• Associate’s degree in Nursing
• Lead: BSN
Licenses and Certification
• Current Illinois RN License
Experience and Skills
• Technical Experience: 3 years
• Lead: 3 years Case Management experience
