Responsibilities

  • Responsible for patient care under the supervision of an attending physician.
  • Discharge planning for patients with diabetes such as: medication reconciliation, dietary instructions, DME orders, referrals, and continuing care instructions.
  • Educates staff on diabetes and current standards of care.
  • Participates in service line planning and coordination.
  • Acts as a liaison with other services.
  • Maintains all required records, reports, statistics, logs, files and other documents as required, including but not limited to payroll, schedules and status changes.
  • Provides diabetes care within their scope of practice.
  • Counsel and instruct patients and significant others on current diabetes standards of care. Delegates diabetes education to diabetes care team as appropriate.
  • Responsible for inpatient diabetes care in collaboration of assigned attending physician.  Works as collaborative partner with diabetes care APN and other diabetes care team members to provide high quality inpatient diabetes care.
  • Initiate referral to appropriate physician or other health care professional of problems that exceed the scope of practice.
  • Makes daily rounds on hospitalized patients and works collaboratively with assigned hospitalist, or other inpatient provider.  Accepts consults for Diabetes Management and diabetes education.
  • Provides bedside assessment on consultations and delegates face to face diabetes education to diabetes care team.
  • Collaborates with but bills independently and separately from inpatient hospital provider.
  • Manages patients with diabetes within recommended standards of care.
  • Provides consultation and recommendations with collaborating providers.

Qualifications

  • Education
    • Master’s degree in Advanced Practice Nursing Studies from CCNEA accredited program.
    Licenses and Certification
    • Current AACC certificate as an Advanced Practice Nurse
    • Advanced Practice Nurse practitioner controlled substance licensure
    Experience and Skills
    • Technical Experience: 2 years current or recent inpatient experience managing diabetes
    • BC-ADM preferred
